Today we met with the perinatologist, which I will refer to as MFM from now on (maternalfetal medicine) because it's quicker to type, for the first time. And had our 20 week level II anatomy ultrasound. AND had bloodwork drawn. All in the same afternoon. I was gone for 5 hours while my sweet friend Virginia stayed at my house and played with all 5 of my kids while our sweet friend Amanda stayed at HER house and watched Virginia's kids so that we could go alone and no one would be at risk of catching last weekend's bug in case it's still lingering around here somewhere. And Virginia scrubbed out my fridge while I was gone.
Isn't that amazing? I know you're here to hear about the appointment, but none of it would have been possible without those two, so....well...super love and prayers for them!
Okay, MFM. I kind of don't know where to start. This might be a long one...
Ultrasound
We didn't realized we'd be getting the full level II anatomy scan today, but we did and we were glad. Do you know what's hard? Using an ultrasound machine with no fluid around the baby. But luckily the u/s tech was pro, pro, pro and managed to find and measure almost every single body part and organ. Incredible. One of the only things she was a little iffy on was, you guessed it, the gender. Dagnabit! But she kept trying and after a bit she said "If you maaaaaaaaaaaaade me guess, like if I HAD to say one or the other right now, I would say this is a boy." Insert arrow onto screen.
And then I was all "I see it! Yes, I agree."
Of course this probably means it's really a girl and she'll throw another loop into this story on her birthday, but until then little dude will be named Nicholas Robert Borobia.
Niiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiiicholaaaaaaaaaaaaaaaaaaaaaaaas! (like those Ricola commercials, claro que si.)
As for fluid levels, they were basically nil. Like, immeasurable at the beginning. But even in the span of the ultrasound, they increased enough that she could have measured almost a centimeter (AFI level of 1). Pee baby, pee!
The other bummer is that he has, until now, measured right on with his gestational age. But now he is starting to measure small because it is difficult for him to grow against the sides of uterus as opposed to growing against fluid- too much resistance against his itty bitty body.
But otherwise he looks totally healthy. Look at all those little organs! Look at that beating heart! Look at all them brain sections! It was so fun to see. I could have watched him all day long. Happy happy.
MFM meeting
After the ultrasound of many many moons, it was time to chat with our new specialist. It's hard for me to remember the order of how things went, so maybe I can just give you my list of questions and the answers he gave afterward? Bueno? And then maybe if one day someone with the same condition (PPROM) is reading this, they can have some questions answered, too.
Q: Is there a cause or a way to prevent subchorionic hematomas, the devil behind the ruptured membranes?
A: Unfortunately, and fortunately, no. You did nothing wrong to cause them and there isn't anything you can avoid doing in the future to prevent them.
Q: What are the best things we can do from now until viability and then beyond?
A: Fluid. Hydration, hydration, hydration. If you had no other children at home to take care of, I'd tell you to lie down during the day. There's no guarantee that will help, but it certainly wouldn't hurt.
(NB: so even though I have 5 children at home, I'm going to do this as much as possible anyway)
Q: What are the health challenges we can anticipate if baby is born after 24 weeks?
A: The biggest concern is going to be respiratory problems. Aspirating the amniotic fluid is mandatory for proper lung development. If baby can aspirate enough in the next four weeks (by week 22), then a steroid injection at 24 weeks will help baby be ready to breathe outside the womb. If the lungs are not physically developed to begin with, a steroid injection won't do anything.
Q: Is there likely a single tear or an overall degradation of the amniotic sac? Is it possible that it will heal itself?
A: When PPROM is caused by a subchorionic hematoma, the reason the sac fails is because your body sends white blood vessels to break down the blood clot. In breaking down the blood clot, the white blood vessels also break down the amniotic sac, which the clot was pressed up against. Because of this, there is probably no chance that the hole in the sac will heal itself.
Q: Will I have to be hospitalized?
A: Yes.
(insert hyperventilation followed by catatonic state here)
Viability day, 24 weeks, is August 2nd. If we make it that far, and I can't imagine why we wouldn't given that we've already made it THIS far, mama (that's me) and baby (that's Nicholas) will be admitted to the hospital until delivery.
Oy.
Two things I do not like: 1) being trapped and 2) being away from my family. I hate that growing in holiness, in any way really, requires doing the things you don't like to do. You don't get fit by lifting weights that aren't heavy for you, ya know? And man....these are my heavy weights for sure.
But we have a good 6 weeks before all that happens. So I have to just take one day at a time. Figure things out. Get things in order. And you know what else? The amazing laundry room project you guys have going on doesn't really seem as much like a luxury now as it did a few days ago. You know why? Because if in 6 weeks my husband is single parenting 5 children (somehow. I don't know how. We'll figure it out.) and people and family and friends and someone is going to be helping with the laundry, I can't really also ask them to empty a dirty water bucket, can I? Or tell them there's no dryer, you know? It's enough that they're going to be doing our chores for us! OR, my kids can actually do the laundry on their own, which they cannot do now with said bucket + potential electrocution situation. Providential, my friends. Providential.
Alright, this is getting long. What have I missed? Oh! We were stoked to confirm that the MFM is proudly a pro-life Catholic- he announced as much during the meeting before we ever mentioned anything about it. That gave me a lot of peace, knowing that he wouldn't bat an eye at us having our priest visit or on hand to baptize the baby or anything like that. Aaaaaaaaaaaand he prayed for us at the end of the meeting. It was very sweet and wonderful and I've just never had anything like that happen before.
So. Deep breath. Heeeeeeeeeere we go!

ReplyDeleteOh we will keep praying! My niece was born at 26 weeks after PROM (though within hours of the PROM). Today she is a small but perfectly healthy three-year-old. Praying the same for your dear Nicholas!

ReplyDeleteOn a separate note: can you take as many horrifying "before" pictures of your current laundry room as possible? So that we can all revel in the amazing amazingness of the afters?
For what it's worth, we set our laundry room up so that we have a white shower curtain bar with white plastic hangers above the machines. This lets me air dry certain things without having to pull out the big clothes rack. We also have a shelf built over the machines that lets us keep the detergent in place when we use the spigit (sp?). And lots of bins for lots of things that we don't want to look at all the time.
Oh, good idea. Maybe even a video? I should take a video, yes.

(And, when we were little, my funny funny father once declared those ricola horns "phlegm horns" - get it, cough drops?! - and made us giggle to our toes - I will never forget it. So I always get a tickle out of seeing the Ricola package or commercial!)
SOOOO happy for you guys, you are going to make it. Hospital will be a challenge for sure - but grace will get you through. SO glad he's healthy thus far!!!

ReplyDeleteHi Dwija. So much good news (in a keep on keeping on way). I just wanted to say that although I was hospitalised with PPROM I was allowed in and out. I was allowed home when I got to about 28 weeks, with a list of conditions, and came in for monitoring as an out patient every couple of days. If anything ever changed they kept me in for a night or two, and when everything was going fine I was allowed home again. Your hospital may be different of course, but there is hope that you won't be stuck there for 16 weeks (because Nicholas is going to make it to 40, no bother). Especially if they know that you have plenty of help at home so you won't be tempted to concrete the driveway or anything. I think it depends a bit on how far you live from the hospital though, as the main danger they're trying to avoid (I think) is having a prem baby born quickly and not in hospital. If it turns out to be a girl will she be Nicola Roberta?
ReplyDeletePraying!! This is what my question was. Why the hospital stay? And why 24 weeks? I know that's considered viability (even though younger babies have survived) so is it just that they want to monitor 24/7 and induce right away if they think baby is struggling? I wonder if it would be safe enough to have something like Melanie where you and baby were monitored intermittently so you could keep up with your vitamin and supplement regime and be at home. Are they worried about labor starting early and you not being able to get to the hospital? Sorry for all the questions...just trying to understand :) Love you!
DeleteDon't apologize! I know how it's easier to pray about something you understand :) So the reason for hospitalization at viability time is so that baby can be immediately delivered or I can be immediately treated at any sign of distress and he will have a better chance of survival. We are "out in the boonies" and I have a history of fast labors, so if there is placental abruption or something like that, getting into the hospital and getting a c-section going would take quite a while. So it's not that being in the hospital will magically help the tear heal but rather that they can tend to him the second that things go south.

ReplyDeletefirst time commenter here (long time reader). I have 6 children. Almost all of my pregnancies were high risk, but I used Lourdes water in all of them. Can you get some Lourdes water for yourself and your baby? This water does work miracles. And since your baby is in need of water, why not give him this miraculous water that our Most Holy Virgin showed saint Bernadete in Lourdes?
I have a little bottle that I could send you. But I am in Brazil and I fear a little bottle of water won't reach you due to US customs issues.
Please, let me know if you can get some of this water. If you can't, I can try and send you a little bottle anyway.

ReplyDeleteOk so here is my story http://lonetater.blogspot.com/2011/11/hardest-thanksgiving.html My waters broke at 23 weeks. Not viable at my hospital. I went 11 weeks with little to no fluids. He was born at 34 weeks. They docs took him then because of another problem I had. I think he would have gone all the way. If you have gotten this far, then chances are great that you will go way long. I had complete rupture of the membranes after leaking for more than a week! I will be praying for you! Bed rest was hard but I did it. I ended up in the hospital for the full 11 weeks 74 days plus 7 days recovering from surgery. Baby stayed 13 days in NICU and is a healthy 5 YO! Blessings to you!

I'll keep the prayers coming, you guys can use them! The low amniotic fluid levels do sound scary but how encouraging to have a faithful doctor and you've made it this far so surely little Nicholas can keep on for the rest of the pregnancy until his little lungs are ready. Keep off your feet and take care of yourself, no lifting buckets for laundry anymore, everyone donating over at Cari's blog has got you covered with a proper laundry room now :)

My daughter's friend's water broke at 20 weeks, she carried her son for another seven and he was born at 27 weeks. He's still in the hospital but is expected to be fine. We're praying for you everyday.
ReplyDeleteHospitalized until delivery does not sound fun at all. BUT you can do it. God will give you the grace you need then, just right when you need it and in the perfect doses. He is the best physician of all. Prayers and hugs.
ReplyDeleteSo glad that the Dr was willing to spend the time with you that you needed/wanted ... but no way is it the longest Dr's appt ever. In 2012 I was diagnosed with Endometrial cancer. My first appt was scheduled for 1:00 PM -- got in to see the Dr at 10:40 PM (Subsequent appointments had me leaving his office at 1:00 AM and 2:35 AM! -- lucky for me I'm a night owl.)

ReplyDeleteThis is awesome news! You can totally do this--one hour at a time. I've got so much hope for your Nicholas. I've seen so many micro-premies really make it thanks to those NICU doctors. So many prayers for the Holy Spirit to protect and heal his lungs. Lots of hugs and encouragement.
